I agree to and authorize the transportation of the shipment as set forth in the Transport Agreement understanding that if I cancel my order within 24 hours of scheduled pick-up time, I may be subject to a TONU (truck ordered not used) fee up to 50% of the original quoted price. I also understand that JMK Transport LLC has a Carrier Down Time rate of $75 per hour if the carrier incurs delays due to customer related issues beginning after 2 hours of downtime. I understand that I will receive an invoice via email from JMK Transport LLC for services rendered with balance due upon receipt agreeing to pay in full within 15 days; failure to do so may result in additional interest/fees which I will be responsible for. I understand that failure to sign this agreement will render the entire transaction null and void.
